Kindergarten Registration Instructions
Forms are currently available through the school office. To request a registration packet, please click here or call the office at 412.833.2433.
All forms in the packet must be filled out in their entirety. Please indicate your preference for full-day or half-day Kindergarten with K(F) or K(H) respectively.
Within the packet you will find the following forms:
- Green Registration form · Please fill out one per student, sign and date;
- Yellow Registration form · Please list all students on one form, and indicate parish, school district, and bus stop on the front. Side two must also be completed, signed, and dated;
- Health History packet · Please fill out one per student;
- FACTS Tuition Management Program contract · Please fill out one per family (see below for more information); and
- Pastor Verification form · Please return one per family, if you are a member of a parish other than St Joan of Arc.
Please complete all forms and return to the school office, along with the appropriate registration fee of $45.00 for one child or $75.00 for two or more children. Note that this fee covers all registrations for grades K-8.
